Guidelines for consent forms
- Parental consent forms will be provided by the OHI team. Additional copies can be obtained via email, website or calling the helpline; contact details can be found on page 23.
- Parents/guardians are required to complete and sign the consent form for each child to participate in the toothbrushing activity. It is recommended that if a consent form is not returned after a week that a further form is given to the relevant parent/guardian.
- Ensure that all completed consent forms are filed and stored safely within the school/early years’ setting.
- All staff involved need to be clear about which children have or have not been given consent to take part in the toothbrushing activity. It may be advisable to keep a toothbrushing register.
